NVLA students needing to obtain their Driving Permit and/or Driver's License MUST obtain a signed DMV-301 Form.

Please fill out the below fields for the attendance office. This information is required to review and determine eligibility.

The student will receive a phone call and/or email within 5 school days with determination and further instructions.

Students have 60 days, from the date of signature, to turn the form into the DMV. Should the form expire, a new request must be submitted and attendance is reviewed again to determine eligibility.
