One vendor registration form per vendor. All vendors must provide photo ID to obtain vendor pass.

Confirmation of vendor registration will be e-mailed to the e-mail address provided within 24 hours.

Phone calls will be made to vendor registrants without e-mail addresses.

This form becomes a contract upon acceptance by the Florida Entertainment Summit. Cancellations must be presented in writing. Cancellations after October 31, 2013 will forfeit any fees. Confirmations will be emailed to the email address provided or a telephone call will be made to those without an email address.
