For and in consideration of the mutual promises and covenants herein contained and for other good and valuable consideration: We, the undersigned, being of lawful age and sound mind, do hereby release (1), the members, staff, and board of directors of said (1), individually and collectively, from any and all claims, demands, rights, action and causes of action of whatsoever kind, nature and description arising from or by reason of any and all known, unknown, foreseen or unforeseen damages and the consequences thereof resulting and to result, from any transactions and other dealings between us and the above named individual attorneys, in connection with: any and all issues, claims, demands, rights, actions, complaints, grievances, and causes of action that relate or may relate either directly or indirectly in connection with (2).
We specifically release (1) from any claim, past, present or future with regard to the above described matter or matters and we specifically and forever discharge the said (1), their successors, heirs, and assigns, and their executors, administrators, partners, agent and employees and their successors and assigns, from any and all actions, causes of action, claims, damages, accounting requirements and expenses of whatsoever kind, nature and description that we, the undersigned had, now have or may have in the future with regard to the above described matter or matters. We further certify that this release has been fully read and understood by us, the consequences of same have been fully explained to us and we execute same freely, voluntarily, under no threat of force, coercion whatsoever and at our own insistence. The words "we" to include both the singular and the plural, their respective heirs successors, and assigns wherever the context requires or permits. It is expressly agreed that this instrument may be treated as a defense to any, action or proceeding that may be brought, instituted, or taken by the parties hereto or in their behalf, and shall forever be a complete bar to the commencement or prosecution of any action or proceedings whatever with regard to the aforesaid matters. LEGAL RACE WAIVER
Please accept my entry in the Children’s Voice: CASA, Inc. I AM FOR THE CHILD Super Hero 5K Run/Walk . I know that running/walking a road race is a potentially hazardous activity. I certify that I have entered this race of my own free will. I am in a physical condition that allows me to enter this event. I agree to assume all risk associated with participating in this event, including, but not limited to falls, the effects of the weather, traffic and the conditions of the roads or paths.
Having read this waiver and in consideration of the acceptance of my entry, I do, for myself, heirs, successors and/or assigns, and anyone entitled to act on my behalf waive, and release Children’s Voice: CASA, Inc. Douglas County Georgia, Georgia CASA, National CASA, City of Douglasville, Douglas County, and all other race workers, volunteers, or sponsors of the CASA Super Hero Run, their representatives and successors from all claims or liabilities of any kind arising out of my participation in this event. My photograph, likeness, or video may be used for legal video may be used for legal purpose by race organizers. I have read this waiver and agree to its terms.
