2. Answer all questions completely and accurately. False or misleading statements during the interview or on this form are grounds for terminating the application process, or, if discovered after employment, terminating employment at any time.
3. This application shall be considered active for a period not to exceed 45 days. Another application must be completed to be considered for employment beyond that time.
4. All job candidates who receive an offer of employment will be required to undergo a post offer drug test before reporting to work. Positive test results will result in a withdrawn job offer.
