If selected to play in the masterclass, I agree to provide a photo of myself and a one-paragraph biography to include listing my age, my teachers, formal schooling, and performing and/or teaching experience for inclusion in announcements. I understand that If I am not selected, my $20 will be applied towards admission to the class as an auditor. I also understand that transportation and St. Louis accommodations will be my responsibility.I also have or will register for the master class on-line through this website by May 5, paying the required $20 application fee.
