The student has a developmental disability, generally evident before age 3,
significantly affecting verbal and nonverbal communication and social interaction.
The deficits are not primarily the result of an emotional-behavior disability.
Deficits have an adverse effect on educational performance.
Lack of instruction in reading and/or math was not a determinant factor in the
eligibility decision.
Limited English proficiency was not a determinant factor in the eligibility decision.
The child is eligible for specially designed instruction and related services.
The parent was provided a copy of the evaluation report AND documentation of eligibility is given to the parent(s).
OR
If parent did not attend the ARC, then a copy of the evaluation report and documentation of eligibility were sent
home to the parent.
