As the parent or legal guardian, I hereby give permission for my child to participate in the Stevendale Baptist Church's ART-is-IN Camp.
I understand that Stevendale Baptist Church is a nonprofit charitable institution. I am also aware that my child will be involved in the normal hazards of theatrical activities. In consideration of acceptance of this application, I waive and release all rights and claims that may arise against Stevendale Baptist Church, and any persons affiliated with this camp/workshop.
I give permission to ART-is-IN Camp and Stevendale Baptist Church Staff to provide and approve immediate and responsible emergency care and transportation should it be required.
I hereby grant Stevendale/ART-is-IN full rights to copyright, exhibit, and publish in any medium including, but not limited to, editorial, illustration, promotion, advertising, internet, or photographs taken by Stevendale/ART-is-IN of my child at participating in the ART-is-IN camp.
