How will the entrances take place into the ceremony? (Will the bridesmaids/groomsmen walk down together, separately, etc)

Will the groom or groomsmen be coming from a separate entrance or from a different direction?

Who will be walking you down the aisle?

How will you and your bridal party be positioned in the front?

Will that posiition change as the ceremony progresses?

Will you be having a unity ceremony? (IE sand, candles)

Will you be doing a grand exit after your ceremony? (Bubbles, Streamers, Sparklers, Bird Seed, leaving in a special vehicle? etc)

RECEPTION

How far is your reception location from your ceremony site?

Where is the parking for the reception location?

Will you have an on-site reception coordinator? If so what is this person's name?

Will you be having a cocktail hour?

Where will your cocktail hour be taking place and do you wish to attend?

Will the photographer have access to the reception set up prior to guests entering?

Are there any specific shots or details during your cocktail hour or reception that you would like taken?

Where will your entrance occur for your reception (ie what door, entryway, hallway, stairwell, etc)?

Is there anything different or unusual about your entrances (IE choreographed intro, fog machines, etc)?

After your entrances will you go straight into… (Toasts, cake cutting, or prayer), etc?

Please list the order of events prior to dinner.

Will you be having a sit down dinner, buffet, family style, just desserts, Hors d'oeuvres?

How many toasts will you be having and who will be giving them?
