Application Information:
Thank you for your interest in the Emergency Medical Institute's Paramedic Program. To have your application considered for this program, you must complete and submit your application along with all supporting documentation.
To be eligible for this program, you must have, and submit:
- Copy of your NYS EMT Certification, or higher.
- Copy of your high school diploma (or equivalent)
- Copy of your government-issued photo ID
- Two Reference Letters:
- One (1) Professional Letter of Recommendation (Individual must have direct knowledge of your clinical practice as an EMT)
- One (1) Personal Character Reference Letter
Please complete all relevant sections of this application as accurately as possible.
Please be aware that your application will not go forward for further review and consideration without all of the above information being provided.
NYS EMS Certification Disclosure
All applicants must sign the DOH‑65 personal affirmation stating whether they have any misdemeanor or felony convictions, as required by 10 NYCRR Part 800. If you do have a conviction, the Department of Health (DOH) will conduct an individual review under Part 800, Article 23‑A, and Corrections Law §753. A conviction does not automatically bar certification, but you may not take the NYS practical or written exams until DOH issues written clearance.
You may attend and complete EMS training while DOH reviews your case.
Providing false information may result in penalties and revocation.
At this time, we are only accepting applications for the January Fulltime "Academy" Program and the Spring 2027 Part-Time Program (April).
