To request an RMA number for Repairs or Warranty claims, please fill out and submit this form. You must have spoken to a ChamSys team member prior to filling out this form or have been directed to fill out the form.
Once you submit the form, we will send you an RMA number and shipping address.
When you have received the RMA number:
- Include your RMA number on the outside of the box/case and on a piece of paper inside the box/case.
- If sending in for repairs, we also ask that you only send in the console. We have all the necessary cables and accessories in our shop.
- If this is a product return, please include all accessories and cables.
- Incoming shipments should be in either 1. Original Manufacturers packaging. 2. Manufactures Flight Case - unmodified. 3. Double-boxed and packaged by a professional shipping provider.

Equipment will not be accepted and may be returned to you without a valid RMA number.
