This is the preliminary information needed to book your date. Once I receive this questionnaire back from you, I will send you the date confirmation. At that point you can make an online deposit to hold your date, and your event will be booked. I will ask for more details about music selection and a few other things in a follow-up Email when you have decided on those things.
