College of Education and Human Services - Western Illinois University
You are invited to participate in a quality assurance and service enhancment survey, designed to aid the College of Education and Human Services in planning for the provision of community outreach "Interactive Innovations" in the form of technology-based community workshops and training. We look forward to supporting area schools, health organizations, parks and recreation organizations, police, fire and first responder services, childcare centers, food service and retailers, non-profit community groups and organizations, parents, children and retirees.
Please take a few moments to provide honest reflections regarding your personal or your organization's technology assistance and instructional needs. This study is conducted by Katrina Daytner, Interim Associate Dean for Administration, College of Education and Human Services and Dawn Sweet, Instructional Development Services Manager, College of Education and Human Services from Western Illinois University.
This study will take approximately 4 to 5 minutes of your time. You will be asked to complete an online survey about your career, field, orgainzation or interests. You will be asked to list areas that you would like to receive technology training or assistance, when you would be most able to attend trainings or events so that we may help you meet your needs, and in what ways you would be interested in participating in future fundraising (where proceeds would be used for items such as, but not limited to, transportation costs for area youth, specific materials for specific topics, and etc.) for community events.
Your decision to participate or decline participation in this study is completely voluntary and you have the right to terminate your participation at any time without penalty. You may skip any questions you do not wish to answer. If you do not wish to complete this survey just close your browser.
Your participation in this research will be completely confidential and data will be averaged and reported in aggregate. Possible outlets of dissemination may be WIU Annual Reporting, Administrator Meetings, COEHS Deans and Department Chairpersons, Faculty and Instructional Development Services staff, the COEHS Advisory Board and Community Stakeholders. Although your participation in this research may not benefit you personally, it will help us collect data to understand the technology and instructional needs that exist within our campus community, and assist us in creating linkages to create a community of sharing. There are no risks to individuals participating in this survey beyond those that exist in daily life.
