Award: Students attending or accepted to an accredited undergraduate four-year institution are eligible for a $1000 scholarship to be paid in two payments throughout the academic year. Students attending or accepted to an accredited undergraduate two-year institution are eligible for a $500 scholarship to be paid in two payments throughout the academic year. In addition, recipients will receive a scholarship to attend the 34th National USHLI Conference in Chicago, IL. Please note that this scholarship is only awarded once, past scholarship recipients are ineligible to apply.
Qualifications: Applicant must be enrolled or accepted for enrollment as a full-time student, in an accredited four-year or two-year institution in the U.S. or U.S. territories, and demonstrate a verifiable need for financial support. At least one parent must be of Hispanic ancestry. Recipients must also be available to attend the 34th National USHLI Conference in Chicago, IL, February 18-21, 2016.
: Applicants must submit a 500-1000 word self-description including: his/her family history (parents, siblings), life and/or work experiences that have influenced the applicant, and pertinent extracurricular and community involvement activities.
Applicants must submit a 500-1000 word analysis that describes the most pervasive issue in their community. How will they address it? How will their education and career experience help them address this issue?

Applicants must submit a one-page resume of their work experience, academic accomplishments and other relevant information.
Applicants must submit a recent photograph (high-resolution, color, size: 2.5 x3.5 ratio, PNG, JPG, or GIF). The picture should be suitable (head shot) for use in any printed literature announcing the recipients.
Applicants must submit a high school or most-recent college transcript verifying their eligibility status and GPA.
Letters of Recommendation:
Applicants must submit three (3) letters of recommendation that meet the following requirements: letters should focus on the applicant’s leadership abilities and past academic performance, while also indicating an ability or desire to effectively serve their community; individuals submitting letters of recommendation should have worked closely with the applicant in an academic, professional, or community service setting. (e.g. high school teacher or counselor, employer, faith-based leader, community leader, professor, academic advisor, coach, etc.); letters should not be written by friends or family members of the applicant.
Deadline: The complete application, which includes, the six (6) requirements indicated above, must be submitted by December 31, 2015 at 11: 59 PM CST. Five of the six required documents will be uploaded within this application. Please be sure to have your three (3) Letters of Recommendation postmarked by December 31, 2015. The application is not considered complete until all required information is received.

Presentation: Scholarships will be presented at the Educational Achievement Luncheon during our National USHLI conference at the Sheraton Chicago Hotel and Towers on February 18-21, 2016.
