IMPORTANT INFORMATION REGARDING DEPENDENTS: The following are the guidelines for each of the plans we administer regarding when coverage ends for dependents. It is your responsibility to ensure you notify Human Resources once your child has reached the age limit or is no longer eligible for the benefit (ie: no longer a FT student).
Medical Coverage- ends end of month in which they turn 26
Aetna Dental- ends end of month in which they turn 26
Delta Dental- ends end of month in which they turn 19 unless FT student, then age 25
Davis Vision- ends end of month in which they turn 19 unless FT student, then age 26
Supplemental Death- ends end of month in which they turn 21 unless FT student, then age 25
AFLAC- ends end of month in which they turn 26

Payroll deductions for Dental Premiums, Supplemental Death Benefit, CNA, AFLAC and Long-term Disability Supplement are deducted in the first two pay periods of the month. All other deductions will be withheld from each check (26 pays).
Additional Important Information and Reminders:
Reminder: Any changes in your family status must be reported to Human Resources in order to keep the Board of Pensions and our optional benefits providers updated. These changes include divorce, marriage, dependent children falling within the age groups outlines above, or spouse/dependents no longer carrying insurance (or have a new insurance carrier) which affects primary and secondary insurance coverage.
