VOLUNTEER WAIVER OF LIABILITY, MEDICAL RELEASE, PHOTO/VIDEO RELEASE AND INDEMNIFICATION AGREEMENT

In consideration for being permitted by Scratch My Belly to participate as a volunteer in various activities, I hereby waive, release, and discharge any and all claims and damages for personal injury, death, or property damage which I may sustain or may which hereafter accrue as a result of my participation as a volunteer and resulting from negligence or misconduct of Scratch My Belly. This release is intended to discharge in advance Scratch My Belly, its’ Directors, Officers, and/or agents to include their heirs, agents, representatives, successors and assignees from and against any and all liabilities, actions, claims, demands, costs, or expenses arising from or in any way connected with my participation as a volunteer and resulting from the negligence or misconduct of Scratch My Belly.

I understand that various activities involving dogs and/or puppies can be hazardous in nature involving the possibility of being bitten, scratched, jumped on, dragged or knocked over. The possibility of serious accidents and/or injury can occur during participation in volunteer activities. Knowing the risks involved nevertheless I have voluntarily applied to participate in volunteer activities and I hereby agree to assume all risks of injury, and to release and hold harmless Scratch My Belly, its’ Directors, Officers, and/or agents who through negligence or carelessness might otherwise be liable to me. I further agree to indemnify and to hold Scratch My Belly, its’ Directors, Officers, and/or agents free and harmless from any loss, liability, damage, cost or expense which they may incur as a result of any injury and/or property damage I may sustain while participating as a volunteer. I agree that all services I provide are charitable in nature and entirely voluntary, and there will be no compensation of any kind for the performance thereof. Further I have been fully advised that as a non-employee I am not covered by worker’s compensation and am not covered under any medical insurance plan associated with my volunteer position with Scratch My Belly.

I hereby grant Scratch My Belly permission to use my likeness in a photograph/video or any photo/video that I upload to the Scratch My Belly Facebook page any and all of its advertising or collateral materials, including the Scratch My Belly website (www.scratchmybelly.org) without payment or any other consideration. I hereby irrevocably authorize Scratch My Belly to edit, alter, copy, exhibit, publish or distribute any photos/videos taken of or by me for purposes of publicizing the Scratch My Belly organization or for any other lawful purpose.
I certify that I am of at least eighteen (18) years old and sign this waiver of my own free will. *
I certify that I am between the ages of sixteen (16) and eighteen (18) years old, and a parent or legal guardian has read the agreement and signed below on my behalf. *
I certify that I have my own health insurance to cover any and all medical bills that I might incur while volunteering both directly and indirectly for Scratch My Belly. *
I certify that I have my own automobile insurance to cover any and all medical and property damage expenses in the unlikely event of a car accident while in the service of volunteering for Scratch My Belly. *
I AM AWARE THAT THIS IS AN AGREEMENT BETWEEN MYSELF (AND MY GUARDIAN, if applicable) AND SCRATCH MY BELLY. BY SIGNING THIS DOCUMENT, I (and my guardian, if applicable) AM VERIFYING THAT I HAVE CAREFULLY READ THIS WAIVER OF LIABILITY, MEDICAL RELEASE, PHOTO/VIDEO RELEASE AND INDEMNIFICATION AGREEMENT AND FULLY UNDERSTAND THE CONDITIONS LISTED ABOVE.

I must comply with SMB policies concerning this pet is in foster care.
I agree to properly supervise said pet at all times. I will be physically present when the pet is allowed outside during which time the pet will either be in a fenced area and/or leashed
If the pet needs medical attention, a member of SMB must be contacted immediately and must approve all vet visits. Said visit must be with one of SMB’s approved vets with whom we work. If FOSTER takes foster pet to an unapproved vet for any reason, FOSTER agrees to be responsible for all medical bills incurred as a result of said unapproved vet visit except in cases of emergency when the pet life may be in danger
This agreement is not transferable. If I am unable to care for the pet I will immediately return the animal to the SMB
I give permission to SMB to enter upon my premises at any time upon 24hrs notice If for the purpose of determining whether or not I am complying with this Agreement, and to remove the pet in the event that I violate the terms of this Agreement
I understand that other animals in my household could be exposed to medical or behavioral conditions that have not been recognized in the foster pet placed with me by SMB, and that SMB is not liable for any disease or injury of my own companion animals or other exposed animal
I agree not to attempt to hold the SMB responsible for any damages which the pet may do to any person or property. I specifically release SMB responsibility pertaining to any damage to property or personal injury or any occurrence relating to the pet and shall hold SMB harmless regarding any damage or injury(ies) of any nature whatsoever, and agree to notify B SMB immediately of any damages and/or injury(ies)
