As a Member of the Continuum of Care (CoC) for Guilford County, I/We:
· Affirm Partners Ending Homelessness as lead entity for the Continuum of Care
· Agree to share data with Partners Ending Homelessness and thus align with the HEARTH Act
· Grant staff of Partners Ending Homelessness access to non-client identifying agency Homeless Management Information Systems (HMIS) data
· Agree to share program information, budget and organizational financial information for purposes of defining cost associated with successful program outcomes
· Commit our agency to the mission of ending homelessness across our CoC and affirm that that is only possible if all agencies work together with this common goal
· Commit to the development and participation in our Coordinated Assessment Process and am/are willing to dedicate staff, resources, and time to this effort
· Actively participate in Partners Ending Homelessness’ meetings, committees, and CoC-wide planning and administrative efforts
· Refer specific "official" questions about our CoC and system to the staff of Partners Ending Homelessness. This ensures a unified voice and minimizes confusion
o Examples: Federal/Local funding processes, system wide data (including Point In Time Count), or specific system wide plans
· Abide by the policies and procedures for operation and governance of the CoC determined by the Partners Ending Homelessness board of directors
· Understand that the goal of Partners Ending Homelessness is to end homelessness in Guilford County. Therefore, Partners Ending Homelessness will advocate for and make decisions in the interest of advancing that goal
· Authorize Partners Ending Homelessness to represent the CoC and enter into agreements (MOU's etc…without obligating individual agencies) to improve the capacity, resources and work of the CoC (i.e. MCO's, Health Systems, Universities, Government agencies)
