  • Please fill this form out at least three weeks prior to your event. 

    Please fill this form out entirely. If there is information you still need concerning time, date, room, please wait until you have it to fill out the form. 

    Once you have submitted this form for approval, you will receive two emails. The first email will come immediately and verify that you have completed and sent the form for approval. The second email will come within 6 business days and inform you of your request status.

    Please be aware that you are responsible for the setting up and tearing down of your own event. This includes putting tables and chairs back in their place, and washing any dishes used and putting them away. A checklist will be included with your confirmation email.
