  • Physical Activity Readiness Questionnaire (PAR-Q) and You

  • Regular physical activity is fun and healthy, and increasingly more people are starting to become more active every day.  Being more active is very safe for most people.  However, some people should check with their doctor before they start becoming much more physically active.

    If you are planning to become much more physically active than you are now, start by answering the seven questions in the box below.  If you are between the ages of 15 and 69, the PAR-Q will tell you if you should check with your doctor before you start.  If you are over 69 years of age, and you are not used to being very active, check with your doctor.

    Common sense is your best guide when you answer these questions. Please read the questions carefully and answer honestly:

  • If you answered YES to one or more questions:

    Talk to your doctor by phone or in person BEFORE you start becoming much more physically active or BEFORE you have a fitness appraisal.  Tell your doctor about the PAR-Q and which questions you answered YES.

    • You may be able to do any activity you want – as long as you start slowly and build up gradually.  Or, you may need to restrict your activities to those which are safe for you.  Talk with your doctor about the kinds of activities you wish to participate in and follow his/her advice.
    • Find out which community programs are safe and helpful for you.

    If you answered NO to all questions:

    If you answered NO honestly to all PAR-Q questions, you can be reasonably sure that you can: - Start becoming much more physically active – begin slowly and build up gradually.  This is the safest and easiest way to go. - Take part in a fitness appraisal – this is an excellent way to determine your basic fitness so that you can plan the best way for you to live actively.

    Delay becoming much more active:

    • If you are not feeling well because of a temporary illness such as a cold or a fever – wait until you feel better; or
    • If you are or may be pregnant – talk to your doctor before you start becoming more active.

    Please note:  If your health changes so that you then answer YES to any of the above questions, tell your fitness or health professional.  Ask whether you should change your physical activity plan.

  • WAIVER/INFORMED CONSENT FORM FOR FITNESS TRAINING

  • I acknowledge that I have been informed of the need to obtain a physician’s examination and dietician’s approval prior to beginning this exercise/nutrition program.

  • I acknowledge that I have voluntarily chosen to participate in this program of progressive physical exercise, which can enhance musculoskeletal and cardio respiratory systems.

  • I fully understand that the personal training, exercise program may be strenuous and choose to participate completely voluntarily. I assume all risk, and accept all responsibility for my health and any resultant injury or mishap that may affect my well being or health in any way.

  • I understand that I am not obligated to perform nor participate in any activity that I do not wish to do, and that it is my right to refuse such participation at any time during my training sessions. I understand that should I feel lightheaded, faint, dizzy, nauseated, or experience pain or discomfort, I am to stop the activity and inform the group fitness instructor. I give NuLife Fitness Camp® and its staff permission to seek emergency medical services for me should I become injured or ill with the understanding that I am responsible for any expenses incurred.

  • I understand that use my photographs or video images are property of NuLife Fitness Camp®. I understand that my image may be edited, copied, exhibited, published, or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image. Photographic and/or video recordings may also be used for the purpose of displaying results. By signing, I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or in public marketing setting.

  • By signing this document, I assume all risk for my health and well-being and hold harmless of any responsibility, the group fitness instructor, facility or any persons involved with this program or testing materials. In signing this document, I acknowledge being informed of the possible strenuous nature of the program. By signing this document, I voluntarily agree to participate in the nutrition and diet plan for this training.  I also grant permission for use of photographs and videos as they are property of NuLife Fitness Camp®. This waiver and informed consent contract will automatically renew itself from month to month from the date of signature.
