For every $5.00 donation, you are eligible for one (1) free engraved name tag, which includes your pet's name and two contact phone numbers.

Due to the various limitations on certain tags - we are refraining from engraving addresses at this time.

If you are requesting multiple tags, you may select more than one option from the drop down menus. Please list all the pet names in the appropriate 'pet name' box and assign the tags to each pet in the free-form comment box at the bottom of this form.
