  • Proof of vaccinations are required:

    We require all client dogs to be vaccinated, and up-to-date, against Rabies and Distemper/Parvo (Distemper/Parvo is commonly listed as DA2PP, DH2PP, DA2PPC, or DAPP). These are routine vaccinations. ADDITIONALLY, if the client dog visits our property, we require [from within the past 12 months] either: 1) a clean intestinal parasite (fecal) test from a veterinarian, or: 2) proof of purchase for a monthly oral intestinal parasite preventative. Fecal results can take a week to process, so please plan ahead.​ Use of a monthly preventative or a clean fecal test must be received before the dog is permitted to visit our property, for any duration of time. Vaccination history and test results (a copy of the vet bill will suffice) may be attached to this form (below), or emailed to me as an attachment (especially if the file is too big to attach below). You can take a photo of your vet bill with your cell phone and attach that photo below (or email that photo), as long as the vet bill can still easily be read. Thank you!

  • Photo and Video Release Policy

  • I hereby assign and grant to The Incredible K9 the right and permission to use, reproduce, distribute, and publish the photograph(s), film(s), videotape(s), audio and video recording(s), electronic representation(s) and/or sound recording(s) made of me and/or my dog(s) at any time during training, boarding, and/or community events, and I hereby release The Incredible K9 from any and all liability resulting from such use, reproduction, distribution, and publication.

    I hereby authorize the reproduction, sale, distribution, copyright, exhibit, broadcast, and/or electronic storage of any and all such photograph(s), film(s), videotape(s), audio and video recording(s), electronic representation(s) and/or sound recording(s) without limitation at the discretion of The Incredible K9 and I specifically waive any right to any compensation I may have for any of the foregoing.

  • General Policy

  • Section 1: The Incredible K9 will endeavor to create as safe an environment as possible for the training and care of the dog(s) identified above (the "Dog"), and will offer only sound, safe, and responsible training, care, and post-­boarding and/or training instructions. Client and/or Owner agrees, however, that all animals may exhibit unpredictable behaviors and that The Incredible K9 will not be liable for the actions of the Dog that are outside of the control of The Incredible K9. Client and/or Owner agrees that the recommendation by The Incredible K9 of any product or service is not a guarantee of performance or Client’s and/or Owner's satisfaction with that product or service.

  • Section 2: The Incredible K9 agrees to indemnify and hold Client and/or Owner harmless from and against any claims of injury, expense, costs, loss or damages caused by the gross negiligence or willful misconduct of The Incredible K9 while the Dog is in the sole custody, care, and control of The Incredible K9. Client and/or Owner acknowledges that The Incredible K9 has no control over the Dog when the Dog is not in the exclusive custody, care and control of The Incredible K9, and Client and/or Owner agrees to indemnify and hold The Incredible K9 harmless from and against any and all claims of injury, expense, costs, loss or damages caused by the actions of the Dog when the Dog is not the sole custody, care and control of The Incredible K9.

  • Section 3: Client and/or Owner acknowledges and agrees that owning any animal, including the Dog, has inherent risks, including but not limited to the risk of dog bites.

  • Section 4: Client and/or Owner fully understands the tools and techniques that are used to train their dog and agrees to the tools and techniuqes which could include: Leash, Prong Collar, Bark Collar, Halti Headcollar, Remote Training Collar, Slip Lead, Pet Convincer, Squirt Bottle, Muzzle, Flat Buckle Collar, or Martingale Collar.

  • Payment and Cancellation Policy

  • Payment Policy: For any and all training programs or boarding reservations, full payment is due prior to, or at, the first training lesson/the first day of drop-off. 


    Cancellation Policy: There is a $50 fee for any lesson or boarding reservation that is changed or canceled within 24 hours. When you schedule a lesson or boarding reservation, you are agreeing that the credit card on file will be charged for any change or cancellation that is less than 24 hours prior to the scheduled lesson or boarding reservation. To avoid charges, appointments must be changed or canceled more than 24 hours in advance, online or by emailing: theincrediblek9@gmail.com. Lessons can occur rain or shine. Training programs must be completed within a specific time period, beginning on the day of the first lesson. The Creating Calmness Behavior Modificaiton Program includes 8 lessons, to be completed with 10 weeks time. Failure of the Client and/or Owner to contact The Incredible K9, LLC to schedule, reschedule, or makeup a lesson for over 4 weeks of time, nullifies any remaining unused lessons, and no refund will be issued. If the Client and/or Owner chooses to cancel training prior to the start date of a Board and Train program, the deposit will not be returned. If the training will be rescheduled for a later date, a new non-refundable deposit must be paid for the new date. If The Incredible K9 feels that the Client and/or Owner will not listen to training advice or follow instructions, The Incredible K9 can, at our sole discretion, choose to cancel the training contract and issue a partial refund during the time of the services.

  • Medical Services Policy

  • As Client and/or Owner, you acknowledge and understand that there are certain risks involved in pet training, boarding, and care. You release, indemnify, and agree to hold The Incredible K9 harmless from any and all manner of damages, claims, loss, liabilities, costs or expenses, causes of actions or suits, whatsoever in law or equity, (including, without limitation, attorney’s fees and related costs) arising out of or related to the services provided by The Incredible K9, LLC.

    When necessary, The Incredible K9, LLC will use 24-hour Emergency Veterinary Service and reserves the right to request such services. If medical care is ever required, and the Client and/or Owner is not present, the Client and/or Owner will be contacted immediately. The Client and/or Owner is responsible for all veterinary charges. In the event that the Client and/or Owner cannot be reached by phone or email, how much money do you, the Client and/or Owner, authorize to be spent on veterinary services? (A number must be entered below. Please do not write "as much as needed". If your budget is unlimited, please write "$20,000" or more).

  • Community Event Agreement

  • After completion of a training program, Client and/or Owner is welcome to join and participate in any Community Events. 

    I, the Client and/or Owner, am aware that there are inherent risks and hazards involved in activities with and around dogs, and if I choose to attend, I am voluntarily participating in these activities with knowledge of potential dangers. I understand that while it is highly recommended for pets to receive necessary vaccinations, my dog may come into contact with dogs who have not received such vaccinations during Community Events. I understand that by attending these events I will take 100% responsibility for my dog's behavior and will not hold Kate Walker, The Incredible K9 LLC, and/or any of its representatives responsible for any harm my dog does to any person or dog. I hereby assume all risk of personal injury to myself, members of my family, guests who may attend, as well as to my dog, while I am attending any and all dog socials or community events sponsored by Kate Walker, The Incredible K9 LLC, and/or any of its representatives.

  • Arrival Agreement

    For times if/when your dog visits Kate's home
  • Kate will meet you outside when you arrive. You can call or text when you arrive: 570-295-5793, or you may just wait in your car until our scheduled appointment time. If you are running more than 5 minutes late, please call or text Kate as soon as you know that your arrival time has changed. Remain in your vehicle, with your dog, until you see Kate exit the house to meet you (due to the nature of a home/residential environment, Kate needs to ensure that all other dogs on the property are inside and secure before training can begin). Please leash your dog before your dog exits your vehicle. Additional specific details (such as food preparation, lesson instructions, etc) will be shared via email or online prior to the day of any lesson or drop-off scenario.

    All services are scheduled by appointment only. Times that are requested outside of the available hours may or may not be available, and are subject to a $50 convenience fee.
