  • INSTRUCTIONS:

    This form is the official student hiring/termination form for the Department of Fine and Performing Arts. This is a required step to hire new student workers, rehire returning student workers, or terminate current employees. It is intended that the student's supervisor and intended student worker complete this information together. This form is to be filled out after a supervisor has chosen a student to work for DFPA. 

    After you submit this Student Employment Form, it will be sent onto the DFPA Hiring Manager(s) for approval. Once approved, you will recieve notification and if further action is needed. 

    If student is a new hire, a new hire packet will need to be completed, and an appointment with Jose Perez will need to be made at least one week prior to the intended start date.  

    If any information has changed including new banking information for direct deposit, or permanent address for tax forms, new documentation should be submitted to Patricia Moore in Mundelein 1200. 
     
    If a student is a rehire, and all of their information has remained the same, this form is the only action that needs to be completed for rehire.  

    Students are not authorized to begin work and should not be scheduled until approval for (re) hire has been recieved. For new hires, all paperwork must be submitted to HR prior to scheduling.

  • Due to the Chicago minimum wage being increased to $13.00 an hour effective July 1, 2019, all new student employees will be hired at the rate of $13.00 an hour. 

  • Student employees are not allowed to work more than 19.5 hours per week total during the academic year. Please select one of the options on how you plan on scheduling this employee. It is the supervisor's responsibility to make sure that students are not scheduled beyond their workstudy allocation. Please keep this in mind when making your selection and scheduling employees. 

  • A note to supervisors: if you are hiring students that work in other department(s), and are using their FWS funds, students may only be available to work limited hours. Please discuss the student's work schedule and hiring arrangements with student prior to hire. Should students run out of FWS dollars, they may no longer be able to work for the DFPA.
