  • Cabinet Restock - Supply Order Form

    PLEASE MAKE EVERY EFFORT TO RESTOCK OFF GOLD CROSS AND LIMIT ORDERING FROM THIS CABINET RESTOCK PAGE. 

     

    Use this cabinet restock page for the times when you:

    1. Are unable to restock from Gold Cross immediately on scene.
    2. You have accessed the restock cabinet for expired medications.
    3. You forgotten to restock consumables from Gold Cross.
    4. For any other reason please be sure to detail why you needed the supplies.

     

    Reminder: If the supplies are not attached to a call number -

    it comes directly out of the SLCFD budget.
