Travel Protection Declined Form I hereby decline to purchase travel protection from All Inclusive Travel Hut doing business as Travel Hut Group. I understand that I may incur fees for travel revisions, travel delay, emergency situations, medical expenses, baggage/personal property lost/stolen, baggage delay, travel accidents, and all other situations where emergency assistance is needed and I accept full responsibility for these occurrences as I am declining travel protection coverage. I understand that additional expenses may be incurred due to unforeseen circumstances and accept the full responsibility of these expenses in the event that they do arise.
I requested that All Inclusive Travel Hut ? Travel Hut Group make one or more travel bookings on my behalf. As the worldwide COVID-19 coronavirus pandemic remains ongoing at this time, I acknowledge that for this reason, and other reasons not reasonably foreseeable at this time, these travel plans may be interrupted or canceled by the supplier that is providing them, a government entity or other third party over which Agency has no control. I further acknowledge that the supplier’s own cancellation, rebooking, and refund policies, subject to any applicable law that is now or may later be in effect, will govern my rights and remedies, including my right to receive a refund, in such an event. Moreover, I understand that should I elect to purchase travel insurance, the terms of the policy will dictate whether, and to what extent, coverage for any financial loss may exist under the circumstances. By signing below, I hereby agree to hold Agency harmless and release it from any and all liability for any damages, including but not limited to monetary losses, I may incur as a result of such interruption or cancellation of these travel plans.
I have the authority to sign this form declining the Travel Protection for everyone on this reservation. IF YOU DO HAVE AUTHORIZATION, PLEASE HAVE OTHER PARTICIPANTS FILL THIS FORM OUT AS WELL.
By checking the box below and submitting this form, I acknowledge that some countries restrict or deny entry to travelers with criminal convictions, including felonies. I understand it is my responsibility to verify entry requirements for all destinations on my itinerary and that All Inclusive Travel Hut LLC / Travel Hut Group and my travel advisor are not responsible for denied entry or related costs. I have disclosed any criminal convictions to my advisor. I accept full responsibility for obtaining any required permits,waivers, or rehabilitation approvals prior to travel (for example temporary resident permits or criminal ehabilitation) and for any costs, fines, detention, deportation, missed travel components, or other losses that may arise from denial of entry. I release, hold harmless, and indemnify the Agency and my travel advisor from claims, losses, or expenses (including legal fees) resulting from my inability to enter a destination due to my criminal history.
