• Music Selection and Ceremony Information Form

    The information on this form will help us to plan our participation in your wedding ceremony. Some of the details may need to be discussed with the minister or officiate. Please fill out this form completely and return at least 1 month before your wedding (2 months before if you want music not in our music list) or we will make your musical selections for you. Please identify requested pieces by title and the catalog number on our website. If you would like recommendations for suitable music, please feel free to contact us.

  • Do you want to hear the whole piece (possibly 3-4 minutes) or do you want us to end soon after the candle is lit/ sand poured (about 1 1/2 minutes)?*
  • Will the guests be dismissed by rows?*
  • Are you willing to pay overtime if guests are not all dismissed withing the hour? (some brides choose to have a CD ready to avoid any overtime possibilities.)*
