Volunteer Consent

Consent:

By initialing below, I agree:

I understand the duties and responsibilities expected of a volunteer.

I will work a specific number of hours each month on a schedule or on a case by case basis as agreed to with Project Transitions.

I will be prompt and reliable in reporting for work and will notify Project Transitions as early as possible if unable to work as scheduled.

I will accept the right of Project Transitions to dismiss any volunteer for poor or inappropriate performance including poor attendance.

 

Liability Waiver:

I do hereby indemnify and hold harmless Project Transitions, Inc. from any and all claims or causes of action that may arise out of performance of my assigned duties. I waive any right of action I have against Project Transitions in consideration of my participation as a volunteer for Project Transitions.

Confidentiality Agreement:

I have read and signed the confidentiality policy and understand that intentional or involuntary violation of the policy may result in termination of volunteer assignment.

I hereby agree to adhere to the above consent, liability waiver, and confidentiality agreement as indicated by my initials.
