• Major/Minor Declaration or Change Form

    1. Complete the following information and click submit.

    2. Print and have it signed by the relevant Department Chair(s) or Program Director(s).
    3. If you are adding a second major you must include a Four-Year Plan with your major form application submission.

    4. Return the signed form to the Registrar's Office, Duffy 112 to complete the process.
