Salary Reduction Agreement Form - 403(b) Plan
By completing this online form, Payroll Operations will be advised of your 403(b) election. The completion of this form WILL NOT result in the establishment of a 403(b) account with your chosen provider. You must establish a 403(b) account PRIOR to completing and submitting this form. The purpose of the Salary Reduction Agreement Form is to allow employees to defer pre-tax money into the university’s 403(b) Plan via payroll deduction. Employees who are currently contributing to the 403(b) Plan can use this form to change the amount deducted each pay or to notify the university of a change in 403(b) providers. If you are separating from the university, you can use this form to elect to defer monies from your sick or vacation payouts you may receive following your separation or retirement.
The university has numerous 403(b) providers available to you. It is your responsibility to establish a 403(b) account BEFORE you submit this form. Failure to establish an account prior to submitting this form may result in your pre-tax contributions being rejected by your intended provider.
