Wedding Questionnaire

Thank you for choosing Amanda Noel Photography. In an effort to make sure photographic services go smoothly on your Wedding Day, please complete the following questionnaire about the details of your wedding and submit within two weeks of receiving.
1. Where will the Bride have her final preparation taking place (last minute makeup and hair finishing touches and getting in the dress) on the wedding day? (Name and Address if NOT the ceremony location).
2. Please let us know if you are getting ready at a separate location than where your ceremony is taking place and if you plan on getting in your dress at the ceremony location.
3. Does your wedding dress have a zip-up back or is it a lace-up corset/multiple buttons:
4. Yes or No: The groom would like getting ready photos taken (can realistically do IF bride and groom are getting ready at the same location).
5. Where is your Wedding Ceremony taking place? (Venue name; address, contact name; email).
6. Approx how long is the drive from the Getting Ready location to the Ceremony location(if different)?
7. Where will your Reception take place? (name; address, contact name; email)
8. Approx how long is the drive from the Ceremony location to the Reception location?
9. Pre-ceremony- typically we will do the guys portraits first to allow the girls to have more time to get ready
10. We usually end all photos a half hour before the ceremony begins. Would you like the photos to end any earlier and if so, at what time would you like pre-ceremony photos completed?
11. Tell us about any restrictions or guidelines at your ceremony location that we should know about (make to sure include if there are any time restrictions as to how early or how late we are permitted to be on the location).
12. Is this a religious wedding?
13. Is there a time set for the Ceremony yet? If so, please list the time.
14. Approximately how long will the Ceremony last?
15. Is there a set time at this point when you need/want to be at the Reception?
16. Do you plan on seeing each other before the Ceremony (commonly known as a First Look)?
17. If you plan on seeing each other before the ceremony, would you like all family photos completed before the ceremony as well?
18. Are there any specific locations that you would like your Family, Wedding Party and/or Bride and Groom photos taken at?
19. Which family group photos would you like taken? (select all that apply - Please allow about 5 minutes per grouping in the timeline)

27. If you are having a formal exit, how will your guests be sending you off for your formal exit? (Suggestion: If your exit takes place after dark it's recommended that sparklers play at least some role in the exit to act as a light source :)Can be combined with other things too).
28. What time does the reception end?
29. Please list your cell, and your fiancé's cell as well as the cell of the best bridesmaid and the best groomsmen to get in touch with on the wedding day, if you two are away from your cell.

38. What are the first and last names of the Bridesmaids? (please list maid/matron of honor first)
39. How many Groomsmen will there be?
40. What are the first and last names of the groomsmen? (please list best man first)
41. Will you have any Flowergirls or Ringbearers? If so, please list their names.
42. Are you having any special activities at the reception we should know about? (ie: dollar dance, anniversary dance, bridal party dance, etc)
43. Please list any specific wedding details you would like us to capture (example: a special guestbook, or your grandma's handkerchief)
44. If you are planning an outdoor wedding, what is the plan if it rains?
45. Is there anything else you would like to communicate with us? :)

47. What is the reception timeline? (specifically what time is grand introduction? dinner? and what reception activities are planned before and after dinner?
48. Would you like to sneak away from the reception and take bride and groom sunset portraits if time allows?
49. Will there be a receiving line or personal row dismissal after the ceremony?
