• Studio 3 Album Changes / Approval Form 2017 (20 sides)

  • ***Use this form to let us know of any Changes and/ or Approvals as we go along. Additional 2 page spreads can be added for $150 each. Additioanl photos (over your package amount) may be added for $5 each. Up to 4 changes are allowed at no charge. Additional changes may incur charges up to $25 per item. There will be a blank form for each round of proofing. We cannot send your album for production until all spreads have been approved.

  • Album Spreads-Approve or Indicate a Change is Needed

    If you are satisfied with a page/spread, mark "Approved". If you mark "Need Change", Describe the change in the appropriate box below. EVERY page must be marked either Approved or Need Change. EVERY PAGE must be marked with your choice (A,B, etc.) in the apropriate box below (even if there is only 1 choice). On your proof, there is a letter after the spread number to be included with your choice. When you album is ready for production, ALL pages musted be marked approved AND have the appropriate letter marked.

  • Masterpiece Prints

    On the first line list EACH of the Masterpiece prits you have ordered with a size and style description, ie 16x20 MP Standout. You will find the description listed below each Masterpiece layout on your gallery. If you are satisfied with a layout, type APPROVED on the second line in the Comments field. If the layout is Not Approved, please describe what needs to be changed. If there are mutilple choices for a given size/layout, you must include your choice (A,B,etc.). Each Masterpiece posted in your gallery MUST be approved before production can begin. Non-Masterpiece prints will not be posted to your gallery and do not need approval.
