In the event your dog(s) has an emergency we will attempt to contact your emergency contact. If that attempt is unsuccessful we will attempt to notify your vet. In the event either attempt is unsuccessful, this form provides Wagg'n authorization to seek emergency veterinary services from the Pet Emergency Center (PEC) in the absence of the owner. Wagg'n will make every attempt to reach you, prior to utilizing PEC.
Pet Emergency Center is a full service small animal emergency hospital seeing both emergency cases as well as less urgent medical and surgical issues. Their mission is to be a caring extension of your family veterinarian by providing the best veterinary emergency and critical care with honesty and compassion.
*Any expenses associated with emergency care deemed necessary for the care of your dog are the sole responsibility of the owner. All costs associated with care provided by (PEC) are to be determined in accordance with service offerings at their facility. Please contact PEC for any questions or to register your pet with them.
I also understand that all dogs must be current on the following vaccinations: Rabies, DHLPP, and Bordetella or a current titering test administered by a veterinarian. Proof of all vaccinations must be provided to Wagg'n prior to admittance to the facility. a. DPP (Distemper - Parvovirus - Parainfluenza) - One or three-year vaccine required b. Bordetella (Kennel cough) - Required every 6 months. Please note, many veterinarians administer this vaccine only by request. c. Rabies - One or three-year vaccine required d. Canine influenza - We do not require, but recommends this vaccination. Consult with your veterinarian for its benefits.
