In order to ensure the best possible data collection with reliable results, we are requiring that the owner/reporter provide personal information in the form of name, email and location.
The NGHWDF values your privacy and will not share or disclose your personally identifying information without your permission. In addition, only a small, limited number of personnel within the NGHWDF will have access to view reports or forms containing personal identifying information.
At the end of the survey, you will be given options to share your information. In cases where you elect to give permission, the NGHWDF will only share reports with other agencies, universities or organizations that adhere to our privacy policies and safeguard your personal information to the same standards.
We have organized the data collection tables by NGSD #. When answering questions, please correlate answers to an individual dog. For example, NGSD 1 would represent Jack and all NGSD 1 responses would be in regard to him; NGSD 2 would represent Jill and therefore all NGSD 2 responses would be in regard to her, and so on. You may report data on living or deceased NGSDs.
If a question is for male or female NGSDs only, keep with correlating numbering system and simply leave the rows for the non-affected gender empty. For example, if Jack is NGSD 1 and Jill is NGSD 2, and the question is for female NGSDs only, you would leave the row for NGSD 1 (Jack) and all other males empty.
If you are reporting on more than 20 animals, take the survey a second time, beginning where you left off (with animal 21). Use the next response box to indicate the group of NGSDs on which you are reporting.
