    Self-Publishing Project Intake Form
  • Congratulations on finishing your book!

    Workaday Press is excited to join you as you enter the next stage of self-publishing — layout and design.

    To provide you with an accurate assessment of schedule and fees for your project, we have compiled this questionnaire for you to fill out. Completing it will give us a better understanding of where you are at in the self-publishing process and how we can best serve you as you move forward.

    Please take note of the following:

    • Make your answers as complete and final as possible. If you have questions, our pre-publication guide is a great place to start!
    • Even though there are required fields, you can move backward and forward on the form as needed. The form will let you know if it is missing a key piece of information required before submission. 
    • You don't have to fill out the entire form in one sitting. If you would like to return and finish it later, you can do so. Click 'Save' at the bottom of any page and Jotform will email you a link to the in-progress form for you to pick back up at the later time of your choosing.

  • Before we begin:

    Have you finalized your manuscript? If not, now is the time. Before layout and design can proceed, it is important that you have taken the time needed to review and make any editorial changes, resolve any lingering misgivings, and proofread carefully for spelling and grammatical errors. These changes become more difficult to make once the manuscript has been formatted. Editorial changes to the manuscript after this point will result in additional charges and delay your book's delivery to fulfillment platforms.

    This stage of self-publishing focuses on how your book will look and feel so that it will be well-received by both readers and prospective readers. To best accomplish this, we require a final manuscript before we begin.

  • Is your manuscript not quite ready? That's okay! We simply ask that you take the time to make sure it is before we proceed with designing your book. When it's ready, simply return to this form and complete it at that time.

    Do you need additional editing or someone to proofread your manuscript? We have an in-house editor with more than 10 years of editorial experience! Select "I would like a quote for proofreading services for my manuscript" from the options above.

  • General Information

  • KDP is a great economic choice for both authors and book buyers. It is highly recommended for your project's fulfillment. If you do not have a purchased ISBN, KDP will provide you with one.*

    Ingram Spark is recommended for hardcover fulfillment and for books with color. To publish with Ingram Spark, you need to purchase your own ISBN or use the free option provided by Ingram Spark.*

    Additional ISBNs can be purchased through myidentifiers.com individually or as multiples. These will need to be purchased by the client before final fulfillment.

    *Free ISBNs are registered to the print-on-demand platform, meaning the platform (KDP or Ingram Spark) will show up as the publisher on your book. For more on ISBNs, please see the "ISBNs + Publication Rights" page in our pre-publication guide.




  • Fulfillment Account Set-up and Credentials

  • To protect your financial information, we require you to set up your own accounts with the fulfillment platforms you have chosen. After you have done so, we ask that you provide your platform credentials. These credentials are required for us to set up your project for self-publishing and will be necessary for you to manage your project after publication has been completed.

    Please Note: This is a secure form. Workaday Press will only use this information for the purposes of account and title set-up.

  • KDP (Paperback and Kindle)

    KDP provides hardcover and paperback print options, as well as digital fulfillment for Kindle files. If you plan to publish with KDP, we ask that you create an account on kdp.com. After you have done so, please provide your KDP credentials below.

    Note: These credentials work for Amazon's Kindle Direct Publishing platform. If you previously had an account for Amazon's CreateSpace platform, those credentials should work for KDP, as well.

  • Ingram Spark

    Ingram Spark provides hardcover and paperback print options, as well as digital fulfillment for .epub files. If you plan to publish in a hardcover format or have a full-color project, we recommend creating an account on ingramspark.com. After you have done so, please provide your Ingram Spark credentials below.

    Note: These credentials work for only Ingram Spark's fulfillment platform.

    If you plan to use publish on any additional fulfillment platform, we ask that you set up your account and provide details on the platform and your credentials below.

  • Interior Information

  • Your book's interior must be printed in full color, which will increase the minimum cost of printing your book. Due to fulfillment restrictions, pages cannot be printed in a combination of color and black and white. 

    If you previously selected for your book to be printed in black and white ink, then please do one of the following:

    • If you wish your images to be printed in color throughout the interior, change your answer to "standard color ink" or "premium color ink" under "I would like my book's pages to be printed in:".
    • If you have decided your images do not need to be printed in color, change your answer on this question to "No".
  • If you do not currently have high-resolution versions of your images, we may not be able to use them in your interior due to print quality minimums. For any desired images, we ask that you attempt to source files of the best possible quality (i.e., the original digital captured image, an image scanned at a high resolution, a purchased high-resolution image, etc.).

    We will review any submitted images and contact you if they are unsatisfactory.


  • Digital Versions

    By default, your formatted manuscript will be converted for Kindle and traditional eBook (.epub) versions. This process will convert your print interior design by preserving the original interior formatting and design as closely as possible for consistency between print and digital versions.

    If you have a more complex project (typically true of reference-style or children's books), you will likely need a custom conversion or a fixed-layout file. Please select the Custom Kindle File option below. We will contact you with a quote.

  • Cover Information

  • What does a basic cover package include?

    • One custom book cover design option with up to three imagery options
    • Use of custom colors and typography
    • Use of a single image—provided by client or selected from stock photography
    • One round of revisions to change colors, fonts, placement, etc. (after the initial image option selection, changes to image are not included)
    • One design presented after the above revision requests
    • Two rounds of minor, non-conceptual changes

    You may supply your own cover. If the cover you supply requires adjustment to be upload-ready for your selected fulfillment platforms, additional fees may apply.

  • Note: For hardcover versions, not all styles are available for all imprint sizes. If your size and preferences do not match up, we will contact you and discuss the available options.

  • For any images that are to appear on your book cover, you must have the right to use these photos or we will source comparable ones. Having the rights for images/files means that you are either the creator of the image/file, you have obtained written permission from the creator of the image/file to use in a book cover design, or you have purchased the image/file from a site that grants you the license to use it. Additionally, any provided images must be a minimum resolution of 300dpi.

  • BISAC Code

    You need to choose a BISAC Code that best fits the genre and category of your book. Go to this website and find the major category for your book. Navigate to the subcategory you wish to select. Copy and paste it in its entirety below. 

  • Distribution Decisions

  • Distribution of Print Book

    By default, your print book will be available for sale through limited online booksellers (and a more extended distribution network if published through Ingram Spark). If publishing through KDP, do you also want to make your book available for purchase to the following?

    • Libraries & Academic Institutions
    • KDP Direct: Make your book available to certified resellers through Amazon's wholesale website.
    • Bookstores and Online Retailers: Make your book available to online and offline retailers such as Barnes & Noble and to other book distributors.

    If you would like to make your book available to these distributions channels, select the expanded distribution option below. 

    Note: If you choose expanded distribution, the minimum price of your title will increase by about 50% to satisfy store requirements for the above channels.

  • Distribution of Kindle Book

    By default, you are choosing to publish your ebook/Kindle book exclusively on Kindle/Amazon.com. This entitles you to 70% royalties of the list price of the Kindle book (as opposed to 35% royalties), which allows you to make more from each purchase of the Kindle book. This default enrolls your book in KDP Select in 90-day increments (automatically renewed). During the time it is enrolled in KDP Select, your digital book cannot be available for sale on any other site. KDP Select requires the price of your Kindle book to be between $2.99 and $9.99.

    Note: You may choose not to enroll in KDP Select. If you choose not to enroll, your only option is the 35% royalty structure.

    Kindle Lending

    Do you want to prevent people from lending your Kindle book? Read this for more information. Note: If you are enrolled in KDP Select, you cannot choose to prevent people from lending your Kindle book.

     
  • Digital Rights Management

    DRM (Digital Rights Management) is a setting you can apply to your Kindle Book that prevents anyone who has the Kindle file of your book from freely sharing it with another user for an unlimited amount of time. However, some authors want to encourage readers to share their work(s) and choose not to have DRM applied to their book(s).

    Note: If you choose DRM, customers will still be able to lend the book to other users for a short period and can also purchase the book from the Kindle store as a gift for another user.

    Important: Once you publish your book, you cannot change its DRM setting.

Client-generated delays in the proposed work schedule may result in longer production timelines. Accelerated/rush projects are subject to designer availability and additional fees.
