ADDITIONAL TERMS AND CLARIFICATIONS
For monthly memberships, a membership period is defined as one (1) month. Memberships are automatically renewed every month unless cancellation is provided at least thirty (30) days prior to your membership renewal period. For yearly memberships, a membership period is defined as one (1) year. Memberships dues are billed each month during the one (1) year period. At the end of the membership period, the membership will be automatically converted to a monthly membership, and the student billed at the then-existing monthly rate unless a new membership agreement is executed. The maT reserves the right to change membership fee schedules at any time.
If, for any reason, you need to stop training, either permanently or temporarily, you understand that it is your responsibility to provide written notice to The maT at least 30 days prior to cancellation or suspension. Yearly memberships may not be canceled early without the express written consent of The maT. Notice of cancellation must be done via email to firstname.lastname@example.org. The notice must say that you no longer wish to be bound by this agreement, and that you would like to cancel your membership. This agreement may also be canceled at any time if The maT ceases operations.
REFUNDS, LATE FEES, AND PENALTIES
Membership fees are non-refundable. Any tuition not received on time will be assessed a $15 penalty fee. Returned checks will be assessed a $25 penalty fee. I understand that it is my responsibility to make sure my payment is received on-time. Should my credit card, bank account, or other form of
payment change, I will notify The maT at least 15 days prior to the next billing cycle to avoid any penalties.
The maT will not send a reminder of your payment being due. Your credit card or bank account statement will act as your receipt of payment.
Membership Rates are subject to change. Rate changes will be communicated in writing, and listed on The maT website located at thematjiujitsu.com at least 45 days prior to implementation. The rate change becomes effective upon the member renewal date (next payment period) following the 45 day
A regular class schedule will be posted at thematjiujitsu.com. The maT reserves the right to make temporary and permanent modifications to the schedule at any time, and for any reason.
Uniforms, equipment, camps, private lessons, etc. are all additional costs and are not included in with tuition or registration. The only required equipment for most classes is tight-fitting athletic clothing suitable for grappling, including (but not limited to) tshirts, rashguards, shorts, and/or spats. Though not specifically required, The maT strongly recommends the use of appropriate groin protection and a tight-fitting mouth guard by all students.
By choosing a membership plan and signing above, I indicate that I have read and understand the terms of this Membership Agreement, and that I have read and received a copy of the Liability Waiver Release Form (available at thematjiujitsu.com). This agreement, and its information, will remain on
file, in a secure location, within The Mat's office and/or digital copies within secure cloud storage online.
DISCRIMINATION AND HARASSMENT
The maT has a zero-tolerance policy for discrimination based upon gender, race, religion, national origin, sexual orientation, gender identity, and the like. The maT further has zero tolerance for sexual harassment of any kind. Grappling requires close contact with training partners, and by signing this
agreement all students agree to not engage in any form of discrimination or sexual harassment while training. Failure to abide by this policy can result in the termination of membership agreements at the sole discretion of The maT.
