This adoption contract is a legal binding contract in which B and L Bullies and Broken Hearts Rescue (BLBBR) is legally lifted as owner of the below listed animal. Ownership is transferred to the above listed adopter once signed and dated.

PLEASE INITIAL THE FOLLOWING
PROPER CARE/ENVIRONMENT: Adopter hereby agrees to care for the animal humanely (including providing adequate food, water, shelter, love and attention). The dog shall reside indoors, in the adopter’s home, and live as a family member and companion only. When outdoors, the dog shall be maintained in an adequate enclosed area or on a leash at all times. The dog shall never be chained to any object or structure. The dog shall not be allowed to roam/run at will.
MEDICAL: Adopter agrees to have the animal vaccinated annually with necessary vaccinations, as local ordinances require. Adopter agrees to provide essential veterinary care including heartworm prevention, flea prevention, and annual check ups. B and L Bullies and Broken Hearts Rescue reserves the right to check with the veterinarian at any time to verify that all basic medical requirements are met. The Adopter agrees to allow their veterinarian to release any and all medical records relating to the adopted dog, at any time. Signature on this contract will serve as release and authorization.
COLLAR/TAGS/LOST/FOUND: Adopter agrees to keep the animal collared and tagged with identifying tags attached. If the animal becomes lost or stolen, adopter agrees to notify BLBBR immediately. Microchips are registered to BLBBR and are transferred to the adopter once this contract is signed.
OWNERSHIP/RETENTION: Adopter agrees not to sell, trade or dispose of the animal. If at any time, adopter is unable or unwilling to care for the animal, adopter agrees to contact BLBBR to discuss further options.
ILLEGAL USE AND TRAINING: Adopter agrees that the animal will not be used for any illegal purposes and will NEVER be fought or used as bait in any illegal dog fighting operation. If this clause is broken, adopter understands that BLBBR will immediately confiscate the dog and all law enforcement agencies will be notified. Adopter further agrees that UNDER NO CIRCUMSTANCES shall the dog be trained or encouraged for the purpose of protection, aggression, or any aggravated display of agitated behavior. Adopter also certifies he/she's residence is not located in a municipality where said animal's breed presence is illegal.
HEALTH/TEMPERAMENT: Adopter understands that while BLBBR makes every effort to place only healthy dogs with sound temperaments, there is no guarantee or warranties regarding the health or temperament of the animal. The dog is adopted “as is” and adopter agrees to be solely responsible for the animal and to indemnify and hold harmless BLBBR from all claims of liability for the conduct of the animal on or after the date of this contract. This Release of Liability and Indemnification shall apply to all known, unknown and unanticipated damages resulting from Adopter’s adoption, ownership and/or control of the dog.
RIGHTS AND AUTHORITY: B and L Bullies and Broken Hearts Rescue reserves the right to make inquiries about the animal at any time. If not satisfied with the condition of the animal or the living conditions, said animal may be removed immediately. Any veterinary services needed due to negligence, lack of care or abuse will be the Adopter’s responsibility and payable to B and L Bullies and Broken Hearts Rescue. *
ADOPTION FEE: The adoption fee is $175.00 and is not a sales price, but rather a donation to offset the medical expenses incurred in the treatment of the animal. The fees are due at the time of receiving the animal and are non-refundable after 7 days from the date of adoption.
