I hereby grant permission for my child to participate in all activities of the Global Education Center during the time my child is enrolled in the Center's Roots, Rhythm & Rhyme Youth Programming. I understand that the Global Education Center staff and teaching artists will supervise my child while keeping his or her welfare as their highest priority. I understand that the Global Education Center, it's staff and it's teaching artists will ne be held responsibles should an accident occur.

I confirm that I have provided all necessary information regarding my child's allergies and special needs. I also grant permission to any staff member of Global Education Center to use reasonable first-aid measures and give consent for treatment in the unlikely event that I am unable to be contacted and a competent medical professional feels that my child needs medical attention. Should my religion prevent medical care, I confirm that I have given explicit alternative instructions for dealing with any and all emergencies.

By agreeing to this waiver, I give the Global Education Center permission to use my child's picture or image to be use for promotion and/or advertising purposes, understanding that at no time will my child's full name be used. If I wish my child to be exempt from being in photos shared publicly, I will sign the appropriate box below.

By signing this waiver, I give permission for my child to perform at any opportunities made available by the Global Education Center with the understanding that I will be informed of any such performance in advance.

By signing this waiver, I commit to insuring that my child attends summer camp as regularly as possible, understanding that too many absences may prevent my child from participating in performances.
