Virtual Visit Report (VVR)
Do not begin this report until you have reviewed the Checklist of Required Documentation and are assured that all documents are easily accessible on your computer. Each checklist item should be uploaded as a single file, preferably in PDF format.
Complete this report if you wish to select Option B, (virtual, teleconfernece review) for your certification visit.
This form must be filled out for each course you wish to be certified/recertified.
If you have additional courses at this location, please be sure to complete this form separately for each course you wish to recertify.
Enrollment Report for Recent 12-Month Period: Provide the enrollment data for the most recent 12-month period that best corresponds to your program year. Your enrollment period may not necessarily correspond to the calendar year. Choose 12 months of the prior year if the report is due before your program ends. Please be sure the enrollment period starts where your last report left off. Meaning, if the enrollment period from your last report (annual or recert) ended December 31, then your enrollment period for this report will start January 1. Refer to your last report if you have questions.
In the table below, enter times as hours and % of hours. For example 5 1/2 hours = 5.5 hours. Round up or down to nearest quarter hour: .25 or .50 or .75 or 1.00.
Note: the Total Student Hours below are automatically calculated using the information above divided by the student to vehicle ratios entered in the previous section, using these formulas:

A maximum of 14 hours is certifiable. Thus, if the total above is greater than 14, only 14 may be used.

You will only be required to submit documentation for those options and variances you are seeking. If you are not seeking any options or variances, you will not have to submit any documentation for this section.
Documentation required for Category 6
The variance option allows a school to apply for a variance of a specific standard that cannot be met in a conventional manner. The school may propose an alternative method of compliance other than described in the PTDI certification Standards. Each request must address a specific standard and will not be given in blanket form for a group of standards. Final determination of a request will rest with the Certification Commission. Requests for variance of a standard are not subject to appeal. Requests for variance are to be made prior to the team visit in order to avoid unnecessary delay in the process or misunderstanding during the on-site visit at the school.
