Your pet is scheduled for an anesthetic/ surgical procedure on ____________ at ___________. If you have already printed or been given the Surgery and Treatment consent or Dental consent forms, the information provided is intended to aid you in understanding options and requirements prior to checking your pet in for surgery. Please read and fill in the requested information. If there are any questions about either form, our staff will answer them at check in or you may contact us via phone at (540) 434-3903 or email at firstname.lastname@example.org.
If someone else is bringing your pet in for you, please send all completed papers with the person at that time. It will save time and confusion. If you do not have these papers, more forms will be provided at check-in.

A rabies vaccine is required by law. Proof of Rabies vaccine, given by a veterinarian, must be provided at surgery check-in, unless previously obtained, or the vaccination will be given on that day. If the vaccine was given here, we already have it on file. If it was given elsewhere, the owner is responsible for providing a copy of the vaccination record/certificate from the previous veterinarian. Records can be faxed to (540) 434-7687 or emailed to email@example.com.
Pre-Anesthetic blood work is required for pets over six (6) years of age and is highly recommended for ALL other pets going under anesthesia.
Withhold food after 10 p.m. the previous night (including the morning of surgery). The pet should have access to water until early morning, the day of surgery.
You have been scheduled at a specific time for check-in/registration. Please arrive at your scheduled time; otherwise we may need to reschedule your pet's procedure.
After your pet’s procedure, you will receive a phone call, between 1:30-2:30 p.m. with an update and an approximate time that they can go home. Your pet may be picked up between 3:30-5:00 p.m., Monday, Tuesday, Wednesday, & Friday. Thursday pick up hours are between 3:30-7:00 p.m., unless otherwise specified.
Please be prompt when picking up your pet.
