By signing this request, the Event Leader commits to conducting an Event in a manner respectful of Creekside UMC and promises a timely return of the designated space and equipment to a clean and orderly state.
Church Key CheckoutIf you have an event that occurs after office hours and you don’t have a key to the church, you must check one out during office hours. Keys can be checked out at the church office to allow entrance for your group. Please use the front entrance only to enter the building. Please make sure to check the key back in during office hours. Office hours are Monday-Friday 9:00am – 4:00pm.
Please do not use the Preschool Covered Entrance except on Sunday. This door that is set up for key pad entrance only. Keys used in this door jam it and make it unusable. Please do not use any checked out keys on the Church House-Upper Level, they will not work.
Mandatory Use Terms *To meet state licensing guidelines, each event will be expected to comply with the following terms of room usage. Facility use is only allowed for the times specified in form. All rooms must be returned to its original layout as is listed on the flier by each door and cleaned. The number of chairs, tables and all supplies, if used, must be replaced before the event is considered completed.
Electronic Signature of Event Leader *Facility reservations are the responsibility of the Event Leader. If this Event is a “Fundraiser”, it must be submitted by Noon of the First Monday of each month and to be approved by the Finance Committee. The submission of this form does not automatically reserve the use of Creekside’s facility and other resources, or guarantee the availability. Once your request is approved, you will receive an email to confirm your reservation.
