Please Note: We will do our best to place you in your first choice department at Ohayocon but this is not always possible due to staffing needs. We want to make the entire convention the best it can be!
All staff are expected to check into the hotel on Thursday and check out on Sunday. If you need to arrive or depart on a DIFFERENT day, please list that here. Early arrivals and late departures will need to be approved by your Department Head.

As a volunteer for Ohayocon LLC, I agree to abide by all applicable rules and regulations of Ohayocon, the Hyatt Regency hotel, the Columbus Convention Center, and the States of Ohio and Kentucky. I understand that I will receive no monetary benefit in return for my volunteer service.
Ohayocon may request a criminal background check from a background check agency. By my digital signature below, I consent to release information concerning my criminal history to Ohayocon.
I certify that my answers on this application are true and complete and that I have not knowingly withheld any information that might, if disclosed, affect my application unfavorably. I understand that any misrepresentation or omission of facts on this form could be cause for dismissal.
I hereby release and waive liability, and any claims or demands, against Ohayocon LLC, its directors, officers, parents, affiliates, sponsors, employees, volunteers, and agents, its successors and assigns, for any injury or illness, accidental or intentional, that I myself or my dependents may suffer while at the event, surrounding areas, and/or traveling to and from the event or in connection with any volunteer work for Ohayocon. Should I, or my dependent need medical attention, I authorize Ohayocon to contact the Hyatt Regency hotel to call an ambulance. Furthermore, I agree that Ohayocon LLC is not liable for any lost, stolen, or damaged property resulting from volunteer work for Ohayocon. I agree that this release is as broad and inclusive as permitted by the laws of Ohio and Kentucky.
