Names are the #1 cause of extra work for our staff.  In the next box, please list your name as you typically would.  If you're Japanese, we understand your family name will be first, followed by your given name.  If you're American, we expect first and last name.

If you use a nickname or handle, please place it in single quotes next-to-last, as below.

If you are affiliated with a studio, please put your studio name here. If you do not participate in a studio, or do not wish this video to be credited to your studio, please leave this field blank.

Video sources also often cause of extra work for our staff.  We don't want you to leave anything out, but you do not need to be specific.  For example, instead of "Pretty Soldier Sailor Moon S" and "Pretty Guardian Sailor Moon R", you could just put "Sailor Moon".

Please list the most significant and recognizable video source first, if one stands out.

Please list audio sources as "Artist - Title".  If the source is a trailer, please list it as "Movie - Trailer".  If the source is an original soundtrack, please list it as "Movie OST - Track".  If your source doesn't fit these formats, please be clear and concise.  Please list additional artists using (feat. ) notation.

Note: We are getting a lot of submissoins entered backwards, please don't do that, it makes more work for us.  Artist first please.

Please do not be concerned that checking any of the following boxes would in any way disqualify your entry.  All entries are accepted to all our contests, and we use the following answers to ensure that only appropriate audiences see your video.
