Thank you for inquiring to foster/adopt an animal from ALL BUT FURGOTTEN, INC.! The following information is requested so that we can ensure the best possible home for our animals but does NOT guarantee approval of foster/adoption application.
In order to be considered, you must: - Be 21 years of age or older.

- Have valid government issued identification showing current residence address.

- Have the knowledge and consent of your landlord,

To ADOPT:
Be able and willing to spend the time and money necessary to provide training, medical treatment, and proper care for an animal.

To FOSTER:
Be able and willing to spend the time necessary to care for an animal.

I hereby agree to all of the above terms and conditions. I have read, or have had read to me, the above agreement which was explained to me by a representative of All But Furgotten Inc. and I agree to be legally bound by the terms and conditions stated. I certify that I am eighteen (18) years of age or older and of sound mind and conscience. I certify that all of the information given to All But Furgotten Inc. in this application is true and correct. I understand that my request to adopt any animal may be denied for any circumstance or situation that could be deemed not to be in the best interest of the animal, or is determined to be in violation of state or local ordinances. I authorize investigation of all statements in this application. I also authorize my veterinarian to release any information that would be helpful in determining my suitability as an adopter to a representative of All But Furgotten Inc.

I agree that if I MUST surrender this animal for any reason after adoption that I WILL surrender the animal back to All But Furgotten, Inc. Board Members and to no one else unless authorized by All But Furgotten Board Members. If All But Furgotten, Inc. has to take you to court to obtain custody of said animal(s) then all court fees, lawyer fees, and misc. fees will be the adopter’s responsibility no matter what the outcome of the case is.
