Education Workshop Request Form 2017-2018
Payment Policy and Procedure:
Workshop fees start at $195.00/hr for a maximum of 30 participants at Flushing Town Hall, $250.00/hr at your site. Please note additional fees may be applied for additional participants.
Please send payments according to the due date (usually 7 days before the workshop). We accept checks, money orders, and credit cards. We also accept DOE Purchase Orders (Vendor# FLU065000). Late payments may result in the cancellation of the program without further notice.
Changing the Dates and/or Times:
You are welcome to change the date and/or time up to 1 week prior to the workshop. Please allow up to 7 business days to change the date. For cancellations less than 1 week, there are no refunds.
If the workshop is cancelled due to inclement weather, we will either reschedule the program or credit you to a different performance and/or workshop.
If you fail to show up on the day of the workshop, you will be charged the full amount. There are no refunds.
