Order Form

1. Complete this order form, including method of payment. Please note, DEPOSIT for all quilts is $100.
2. Print this form and, if not paying by check, print the deposit payment invoice you will receive by email.
3. Box up your shirts. NOTE: Do NOT cut shirts. Seal all shirts in a plastic bag and include the printed deposit payment invoice or your deposit check and a copy of this form.
4. Send your shirts to: 310-D Wood St., West Monroe, LA 71292 or call 318.307.1911 to schedule drop-off at our location.
5. When your quilt is near completion, you will receive an invoice for the remaining balance. You may pay by check, credit card or PayPal.
6. After payment is received, your quilt UPS tracking number will be sent by email.
