VERIFICATION: I certify that my answers given herein are true, accurate and complete. I understand that if I provide inaccurate or incomplete information, I may be declined any offer of employment.
NO OFFER: I understand that neither this document nor any offer of employment from Joseph Productions, Inc. (“JPI”) constitutes an employment contract unless a specific document to that affect is signed by JPI’s President.
AUTHORIZATION AND RELEASE: I authorize JPI to verify the information contained in this application and to investigate my personal or employment history. I also authorize any former school, employer, person, corporation, credit agency, or government agency to provide information that they may have about me to JPI. I release JPI and all other providers of information from any liability as a result of furnishing and receiving this information.
PRE-EMPLOYMENT BACKGROUND CHECK: Any offer of employment at JPI is conditioned on passing a pre-employment background check including criminal history, credit history (where applicable) and driving record. As a condition of employment, you agree to cooperate and provide any necessary authorizations and releases of records.
SUBSTANCE ABUSE / DRUG-FREE WORKPLACE: I understand that JPI is substance abuse / drug free workplace. Any offer of employment is conditioned on passing where applicable: a pre-employment physical examination; and, drug / illicit substance screening. Employees shall not: report for work in an impaired condition resulting from the use of alcohol or drugs / illicit substance; consume alcohol while on duty; or unlawfully possess or consume any drugs or illicit substance while on duty, at a work site or in the course of employment.
LIMITED DURATION OF APPLICATION: This application for employment shall be considered active for a period of time not to exceed sixty (60) days. Any applicant wishing to be considered for employment beyond this time period should inquire as to whether or not applications are being accepted at that time. The terms and conditions provided herein shall survive indefinitely.
WARRANTY OF INFORMATION: In the event of employment, I understand that false or misleading information in my application or interview may result in my discharge. I also understand that I am required to abide by all rules and regulations of JPI and that employment is subject to conditions and policies in the JPI Employee Handbook including any amendment thereto.
“AT-WILL” EMPLOYMENT ONLY: I understand that any employment by JPI is "at-will." This means that either an individual employee or JPI may terminate the employment relationship at any time, with or without cause, with or without notice, for any reason or for no reason at all.
DISPUTE RESOLUTION: In connection with this application for employment, and any subsequent employment by JPI, I agree that any dispute between me and JPI (including any employees, managers, officers, directors, owners, customers, and/or vendors of JPI) must be filed with the American Arbitration Association for binding arbitration or in a court of competent jurisdiction with the right to trial by jury being expressly waived by all parties. Further, any such disputes must be filed, if at all, within six (6) months of the claim first accruing or shall be time barred.
