Hey CAACURH! Thanks for your interest in purchasing CAACURH T-Shirts! Please only have one person per insitution fill out this form. Once it is filled out, the ADAF will send an invoice to the email address(es) you provide. Please look over the invoice and make sure everything is correct. If you need to make a change to your order, please email Gregory Vass, the ADAF at email@example.com and a new invoice will be sent to you. The deadline to fill out the order form and submit any changes is Friday, September 29th at 11:59 PM. Each T-Shirt is $10. However. you also have the option to get a t-shirt at RLC without pre-ordering but those will cost $12 each.
We will be handing out t-shirts as schools sign in on Friday, November 3rd. Please come with either cash, chech, or credit card. If paying by check, please make the check out to CAACURH.
