  • Joliet Public Schools takes bullying very seriously. 

    If you, as the student, or your child has been subject to harassment, intimidation, and/or bullying by another student, we encourage you to complete this form providing details of the incident(s)

    This form acts as initial correspondence and/or additional information upon which Joliet Public Schools may use to assist and remedy an incident(s).

    This form should only be completed by a student or on behalf of a student by a legal guardian pertaining only to student to student harassment, intimidation and/or bullying.

    Guidelines:

    • Complete for Student-to-Student incidents only.
    • Provide as much detail as possible, including date(s) and time(s).
    • Provide accurate contact information.
    • Anonymous submissions will not be reviewed.
    • Form will not be reviewed if the accused is a school staff member. 

    Parents or adults seeking to open communication regarding an incident pertaining to harassment, intimidation or bullying against a school staff member must contact school administrators by clicking here.

    Reporting Person Information:
    No disciplinary action will occur on the sole basis of this report. The information submitted is confidential and will be reviewed by Joliet Public Schools Administration.
