The minor is physically able and mentally prepared to participate in all activities as described in the announcement for the program. Acknowledge that I have read this document,I have inspected the facilities and equipment, I accept them as being safe and reasonably suited for the purposes intended, and I voluntarily sign this document. I assume full responsibility for, and risk of, bodily injury, death or property damage due to negligence of releases or otherwise.2020 COVID-19 Waiver and Liability Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19 The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. San Diego Jaguars (Coach Ramon Vasquez) has put in place preventative measures to reduce the spread of COVID-19; however, the Program cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending the program could increase your risk and your child(ren)’s risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending and/or participating in the Program and that such exposure or infection may result in personal injury, illness, permanent disability, physical and mental harm, pecuniary losses, medical expenses, loss of income, and death. I understand that the risk of becoming exposed to or infected by COVID-19 I the program may result from the actions, omissions, or negligence of myself and others, including, but not limited to League employees, officers, volunteers, coaches, managers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any and all injury and losses to my child(ren), myself, friends, or family members including, but not limited to personal injury, illness, permanent disability, temporary disability, physical and mental harm, death, economic losses, medical expenses, loss of income, and all other damages, losses, and expenses of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s or my attendance at the trainings or participation in programming (“Claims”). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, forever discharge, and forever hold harmless the San Diego Jaguars and Coach Ramon Vasquez, all sponsoring agencies, all sponsors and advertisers, all other participants of the program, the owners and lessors of the premises used to conduct programming, and all of their officers, officials, employees, volunteers, agents and representatives, and all other persons and entities, of and from all Claims and losses, including all liabilities, claims, actions, causes of action, losses, harm, injuries, damages, costs and/or expenses of any kind arising out of or relating to the attending or participation in the program and program events. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the San Diego Jaguars, its employees, agents, volunteers, officers, and representatives, whether a COVID-19 infection occurs before, during, or after attending or participating in any League program.
