By signing below, you indicate that you have read all information regarding the 2017-2018 Danceline Elite Trips and understand the requirements thereof. I understand the Cancellation and Customer Payment Policies. If Elite Trip is paid in full at time of registration, the traveler may receive refund for balance less the non-refundable deposits and processing fees ONLY if WRITTEN notice is MAILED via regular post office mail to the Danceline Main Office at least 10 business days prior to December 1, 2017. Faxes, e-mails or voice messages are NOT accepted as cancellations for refunds. Refunds are not granted until after the conclusion of elite trip season.
I understand that the first two payments are non-refundable deposits.
I agree to follow all rules for Danceline Elite Trips and the site on which the Danceline event takes place. Danceline company and all individuals serving as Danceline staff will not be held responsible by me, or any of my participants or by parents or legal guardians of my participants for any accident that may occur during the event.
I give my permission for Danceline to use performers, choreographers and teachers likeness in photographs and videos/DVDs for publicity purposes.
I understand that if final balance for the Elite Trip is not paid in full based on payment schedule, then a late fee of $50 per week will be added to the final balance.
I also understand that all travelers that are considered a student, dancer, child or manager under the age of 18 years old are required to have an adult chaperone of 21 years or older attend with them.
