You must complete a separate form for each course you are appealing. Appeals must be submitted no later than 4 weeks after the term has ended.
You cannot submit this form until your grade has been finalized.

Please select the correct department based on the class's course prefix (BIB, BUS, etc.) for which you are submitting this appeal. Selecting the appropriate department ensures that your request is directed to the correct department for review. Appeals submitted with incorrect or missing course prefixes will not be processed.
List every absence date separately and provide a brief explanation of the absence. All absences must be included, including absences for school activities. Example: athletics, Lifesong, etc.
Incomplete forms are subject to automatic denial. Allow a minimum of 7 business days for review. All correspondence will be sent to your student email account.
