Staff Registration Form Logo
  • Welcome Staff to the 2021-2022 Season

This registration session will allow you to complete contact information, sign up for participation and submit consents.
  • USPSA Registration

Conference Team realignment will be based on the results of the 2019-2020 season competitions as well as other factors. The complete team conference alignment will be updated after the registration deadline has ended on December 31st.

There are three levels of involvement for power soccer teams. The 2021-22 season game requirements have been decreased to minimize the amount of travel during the pandemic.

    • Conference: Teams that want to be in the competitive level of Conference play must play a minimum of 8 games total. The 2 game requirement against teams in the same conference or higher has been removed for the 2021-22 season.

    • Non-Conference: Teams who don’t wish to be quite as competitive or travel should register as a Non-Conference team. Non-conference teams can still play competitive, sanctioned USPSA games, but there is no minimum number of games to play and no end-of-season tournament.

    • Recreation: A Recreation Team is for a team that has a program but doesn't wish to play competitive games but wants to stay informed about the sport and provides a path to those players who wish to move to competition.

Before you play any games, remember – you and your team need to register! Registration is done online RIGHT HERE!

    Registration Requirements
All head coaches, assistant coaches, and other team staff members must submit the Registration, Code of Conduct and Waiver/Liability Release forms online. New staff members also need to submit a physical copy of the Waiver/Liability Release.

    Waiver/Liability Release

  • Code of Conduct

  • In consideration of the participation in activities of the USPSA, membership on teams organized, sponsored or sanctioned by the USPSA, and participation in competitions (international or domestic), camps, clinics, exhibitions, programs or appearances for, on behalf of, or as a member of the USPSA, I acknowledge, understand, and agree to abide by this Code of Conduct and pledge to uphold the spirit of this Code of Conduct, which offers a general guide to my conduct. Therefore, I pledge and agree to the following:

    1. I will at all times display the conduct expected of me as a member of the USPSA and conduct myself in a manner that will not in anyway bring disrespect, discredit, or dishonor to either myself, my teammates, the USPSA, my country or organizer of an event in which I participate.

    2. I will conform to all applicable local, state, and federal laws and regulations, and any rules, regulations, and codes that may be established for events, programs, and activities in which I participate, including those of the USPSA, Federation International of Powerchair Football Associations (FIPFA) and any organizer of an event in which I participate.

    3. I will act in a sportsmanlike manner consistent with the spirit of fair play and responsible conduct.

    4. I will respect the property of others whether personal or public.

    5. I will respect members of my team, other teams, spectators and officials, and engage in no form of verbal, physical or sexual harassment or abuse.

    6. I realize that if I choose to take actions other than those described herein, I will be subject to disciplinary action and that the consequences of my actions could possibly affect my USPSA membership in the future. In addition to the foregoing, but not by way of limitation, the following could result in disciplinary proceedings:
      1. Any physical damage to facilities, equipment, furnishings, or loss of items in a room where I am lodged, which will be paid for by those individuals assigned to the room in which the damage or loss occurs, including destruction of property (including that caused by horseplay, fighting, or willful misconduct etc.).

      2. Any act considered to be an offense under federal, state, or local laws; or a violation of the applicable rules, regulations, or codes of the USPSA or the organizer of an event in which I participate.

      3. Misconduct (i.e., inappropriate horseplay, theft, fighting, etc.).

I recognize that this Code does not establish a complete set of rules which prescribes every aspect of appropriate behavior.

I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual, significant hazard during my presence or participation, I will cease participating and bring such hazard to the attention of the nearest official immediately.

In the event that I am unable to do so because of an injury or illness, I hereby consent to the administration of first aid or other medical treatment. I agree to assume full responsibility for payment of any and all fees incurred as a result of such medical treatment. I understand that all participants in the Programs are required to have their own medical insurance coverage, and that neither United States Power Soccer Association or any other sponsoring entity provide such coverage.

I hereby voluntarily and without compensation authorize visual images and/or voice recordings to be made of me by or on behalf of United States Power Soccer Association, and other sponsoring entities during the Programs. I also authorize the foregoing entities and their assigns to reproduce, modify, publicize, broadcast and display any such visual images or voice recordings, with or without my name, without notice or payment of any royalty, fee, or other compensation of any character to me for the use of my image, name or voice.

I hereby covenant not to sue and release the Releasees and their employees, contractors, licensees and assigns from and against any and all claims that I may have for invasion of privacy, right of publicity, defamation, copyright infringement, or any other cause of action arising out of the use, adaptation, reproduction, distribution, broadcast, or exhibition of my likeness, name or voice.

This covenant not to sue, release and hold harmless agreement is binding on me, my heirs, assigns, personal representatives, administrators, and next of kin.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT INDUCEMENT.
